People receive support after an assessment has been completed by a nurse or social worker in the Community Support Team.
During the assessment, they will discuss with you (and your carer/s) any difficulties you may have and what kind of support will be most helpful to you. There is no charge for assessments.
A set of rules called 'eligibility criteria' will be used to make sure that priority is given to people with the most urgent needs or who are at greater risk of coming to harm (for further information see our separate leaflet on eligibility criteria).
If you are assessed as needing support and services, this will be written down in a care plan and you will be given a copy to keep.

Some services require payment of a charge, although charges are usually based on the ability to pay.
