Hello, I would like to tell you a little bit about the Community Support Team.
The team is made up of social services staff who work for the City and County of Swansea and health staff who work for Bro Morgannwy NHS Trust. They are based at Herbert House, on the Enterprise Zone quiet near to the Ramada Jarvis Hotel.
In the team there are social workers, community nurses, speech and language therapist, occupational therapists, physiotherapists and psychologists.
The team also work closely with a consultant psychiatrist for Learning Disability Services and also the Community Response Team.
We work with adults with a learning disability to help them identify their needs and the best way of meeting these needs. In fact, if you receive any of the services outlined on this website you will have a nurse or social worker, called a care manager. This is because in order to access any of the services you will need to have had an assessment carried out by a care manager.
