Where do we work?

City and County of Swansea

Our geographical area of operating is the City & County of Swansea.

How do we provide your support?

Your Care Manager sends a referral to Doorways

We meet with your Care Manager

We have a meeting with you, your carer and your Care Manager, so you can tell us how you would like support

We provide your service as we agreed

We will review it at the agreed time

We ask you to complete a questionnaire to tell how happy you are with your service

How do we check your service is good?

  1. We will agree with you what help we will provide at a Service Agreement Meeting.
  2. We will meet with you at least every six months to see how our support is going. However you can ask to review your agreement at any time.
  3. If your needs change or circumstances change, we will meet to see what else is needed.
  4. When you finish with our service we will complete a form with you to see if you think we did well or what we did not do so well. Every year we will complete our Satisfaction Questionnaire with our service users.
  5. Your support worker will have supervision with their manager once every 6 weeks to ensure things are going okay and to have help and advice if needed.
  6. Your support worker will go on courses to make sure they know how to help you.
  7. We will check to see if we need to bring any equipment to help you following a risk assessment. We have some things we can bring but if you need anything to help us help you, we can talk about this with your Care Manager.
  8. You must make sure our workers are safe by making sure that your home is safe and you are not unkind or hurt them.
  9. Once a month our Support Service Leader Adam meets with Stuart Harper, the Manager of the Community Support Team of Social Services. Adam prepares a report for this meeting and Stuart checks if Doorways are doing what has been asked of them.
  10. We have a Service User Committee. This group is helped by someone who is not part of Doorways. The Group Organiser attends the Management Committee Meetings every three months or when there is something they would like to chat about more urgently. The group meets about every 2 months. The Committee is made up of 8 representatives. If you would like to join the Committee or would like to inform the Users Committee of anything you are happy or unhappy with, please ring 01792 476132 and we will arrange for you to speak to the Group Organiser.
Doorways Support

What are your Rights and Responsibilities when having help from Doorways?

You have the right for us to be flexible and alter our help as needed, but this may not be possible straightaway if many issues need to be discussed.

Comments

You have the right to make comments on our service at any time.

To be treated kindly at all times.

To request a review of your support.

Have the help we agree at your service agreement meetings. If the help you need changes we will meet with you and your Care Manager to talk about it, but we may not be able to change things until this happens.

Make choices

You have the right to make choices about your service and be asked how you feel.

When having help from Doorways you must not treat staff unkindly or hurt them.

You should be in when your support worker calls. Please phone if you can’t be there.

If for any reason we cannot provide the help you need, we will tell you and your Care Manager as soon as possible. Your Care Manager may then ask another service to provide you with help.

We will risk assess your sessions. If there are any dangers we will reduce these dangers as much as we can by looking closely at how we can best provide your support.

Work together

We will do whatever we can to look after your safety when we are helping you.

We will ensure that we provide the equipment necessary to help you. If you need to supply it we will let you or your Care Manager know. In such circumstances we may not be able to help you where this equipment is not available.

Doorways have a range of Policies and Procedures to ensure that we provide your support in the best way we can. We review these every year. You can request information on any policy. Just ring us at Doorways and we will supply the information in a style that suits you best.

Some of our key policies and procedures are:
