Make a complaint I can also use this section to keep a record of any requests that I have made of the Consortium and any compliments, concerns or complaints that I have made to them.

Consortium staff will help me to understand this information and make the most of my support from the Consortium.

 

Support to help me Live in my Home

To live in my home my landlord says I must have support to help me manage the things I need to do. 

Some of these things are:

I might need support to live my life.  Some of the things I need help with might be:

Doing the things I choose and enjoy

Choosing who provides my support

I can choose who supports me to:

I do not have to have the same people to do both

If I want to I can choose to have different people, who do not work for the Consortium

If I do not choose the Consortium to provide my support, it does not mean I will have to leave my home.

How do I decide?

Social services

I can talk to my Social Worker or Community Nurse who will help me decide, and tell me about other people who can support me.

 

 

Talk to my family I can also talk to my family, friends or an independent person.  These people are often called advocates.

 

 

 

Support

I will need to tell my Landlord if I want to change the people who support me to live in my home.

Before any change can happen, my Landlord will have to agree.

 

Consortium Support

If I have chosen (or the Social Services Department have asked) the Consortium to help me live in my own home or to live my life, it means the Consortium is my Support Provider

The Consortium expect the staff who support me to be helpful, polite and listen to me.

I have a right to have a say in which staff work with me in my home, and can help to choose staff if I want to.

I will have a named keyworker.  My Keyworker is someone who will take a special interest in me, and help me.

Other Managers who help me have good support:

My Keyworker’s boss is called the Service Co-ordinator.  The Service Co-ordinator manages my support staff and makes sure I get the support I need.

My Contract Manager has overall responsibility for the staff and managers who work with me.  They have to make sure The Consortium work with me in the way my Social worker has agreed.

How the Consortium will support me?

I can choose to have:  (Please decide with your Social Worker, Community Nurse or close relatives, which of these you want).

A Support Contract telling me about the Consortium and how they will support me.

A Service Delivery Plan to make sure that the service meets my needs.

A Service Review to make sure I am getting a good service.  Other people may also be asked if they are happy with my service

Participation Agreements that says how the Consortium will help me have my say and agree about who else we will involve.

I will be invited, together with the other people who support me, to a my Service Review that will take place every 6 months.

In the Service Review I can have my say about how well I think my service and support is going.
