Community Support Team for People with Learning Disabilities

Who is this leaflet for?

This leaflet is for you if you have learning disabilities. This means that you may have difficulties in learning and using the skills needed to lead an ordinary life without the support of other people.

These difficulties would have arisen during childhood and most often you would have been born with the disability. You may also have another disability such as epilepsy, physical disability, sensory impairment or mental illness.

This leaflet is also for you if you are caring for someone with learning disabilities.

What is this leaflet about?

This leaflet is about the Community Support Team and how they can help you to receive the support you need to live as independently as possible in the community.

Who is the Community Support Team?

The team is made up of social services staff who work for the City and County of Swansea and health staff who work for Bro Morgannwg NHS Trust. They are based at Herbert House.

In the team there are social workers, community nurses, psychologists, speech and language therapists, occupational therapists and physiotherapists.

The team also works closely with the consultant psychiatrist for Learning Disability Services and The Community Response Team based at Llwyneryr Hospital.

What help and support can we give you?

We will help you identify what your needs are and the best way to meet them.

In order to do this a nurse or social worker, called a care manager, will come and talk to you and your carer or advocate. They will spend time getting to know you. This is called an assessment.

Your needs and how best to support you will be agreed with you and your carer or advocate. This is written down and called a care plan. You and your carer will be given a copy of the care plan to keep.

We will meet with you and your carer at least once a year to check how things are going and to see if you are happy with the support you are getting. This is called a review meeting.

After this a new care plan will be written. The review meeting can take place earlier than 12 months if anyone, including yourself, feels that your needs have changed significantly.

How to complain

We will be happy to discuss with you or your carer any concerns you may have about the service. The Social Work Team Leader or Health Manager can be contacted at the team. If you are still not happy, the team will encourage you to use the Social Services Department's or Bro Morgannwg's Complaints procedures.

Information sharing and confidentiality

All personal information about you will be treated as confidential by the CST and will only be shared with others where appropriate and with your permission.

Within the CST, health and social services staff work closely together and share information in order to provide a more effective service. This includes a joint computerised information system and joint paper records.

Both the City & County of Swansea and Bro Morgannwg NHS Trust operate an access to records policy which enables people to see information held about them.
